If you have health insurance coverage and receive a bill from us that does not indicate that an insurance claim has been filed, it is probably because we are not aware that you have insurance or do not have information necessary to file a claim. Insurance Claims: We are happy to file your insurance claim for you. Click here to download our Private Insurance Form. Once you've completed the required information you may submit it using one of the following methods:

Medicare: If you are a Medicare Part B beneficiary we will file a claim on your behalf. You do not have to pay anything upfront for covered services. If Medicare determines that the service provided is covered they will pay 80% of the allowed amount to us, minus any unpaid deductible. You or your secondary insurance company are responsible for the 20% co-pay amount.

Medicaid: If you are a Medicaid recipient we will file a claim on your behalf. Emergency ambulance services are covered by all forms of Medicaid in Indiana. Nonemergency ambulance services are usually covered by traditional Medicaid but not by most managed care Medicaid plans.
